Why the synergy works
Each tool addresses one piece of the regenerative puzzle:
- Microneedling creates thousands of micro-channels through the epidermis into the dermis. Each channel triggers a local healing response — the "controlled injury" mechanism that drives collagen formation.
- PRF provides the growth-factor signal that tells the responding cells what to build, for how long, and at what intensity.
Microneedling alone: collagen response is real but the signal is short-lived — the wound heals quickly and the rebuilding phase shuts off.
PRF alone: growth factors are present but their access to the deeper dermis is limited; they sit in superficial tissue.
Combined: PRF growth factors travel down the microneedling channels into the dermis, where the controlled-injury response is active. The signal lands precisely where the work is needed, and the 10-day release window matches the active rebuilding phase.
Best indications
- Atrophic acne scars — the strongest indication. Multi-session series usually meaningful improvement.
- Photoaging skin — overall texture and tone
- Stretch marks — particularly newer (red/pink) marks; mature white striae respond less
- Fine lines and crepey skin — especially around the eyes, mouth, neck
- Pore appearance — through dermal remodeling and skin thickening
- Surgical scar revision — for soft, hypopigmented scars
How the session runs
- Numbing — topical anesthesia 30–45 minutes pre-treatment.
- Blood draw for PRF preparation — 20–30 ml.
- i-PRF prepared while numbing takes effect.
- Microneedling pass across the treatment area — typically 1.0–2.0 mm depth for face, deeper for scars.
- PRF application — topically across the freshly microneedled skin (where channels are open) and/or injected superficially into deeper scar areas.
- Light pressure and short wait for absorption.
- Aftercare: avoid sun, harsh products, makeup for 24–48 hours.
Downtime
- Day 0–1: skin red and slightly swollen, like a moderate sunburn. Tight feeling.
- Day 2–3: mild flaking. Makeup can usually be applied from day 2 with gentle technique.
- Day 4–5: back to normal appearance; tiny pinpoint bruising in some patients.
- Week 1–2: gradual improvement in skin tone becomes visible.
Strict sun protection is essential for 2 weeks post-treatment. UV exposure on freshly microneedled skin can produce hyperpigmentation.
Number of sessions
For skin quality / fine lines: 3–4 sessions, 4–6 weeks apart.
For acne scars: 4–6 sessions, 4–6 weeks apart. Sometimes longer series for severe scarring.
For stretch marks: 4–6 sessions, 4–6 weeks apart. Newer marks respond better than mature ones.
Maintenance: 1–2 sessions per year after initial series.
FAQ
Can I just do microneedling without PRF?
Yes — and it has real effects. Adding PRF amplifies them. For mild concerns, microneedling alone may be enough. For acne scars or significant skin-quality issues, the combination is clinically better.
Is the combination more expensive than microneedling alone?
Yes — the PRF adds the blood draw and processing. Cost increase per session is moderate; the result-per-session is higher, so total cost-to-result is often similar or favorable to microneedling alone.
Are there any contraindications?
Active acne breakout (treat the acne first), active skin infection in the treatment area, recent isotretinoin (6 months wait), blood disorders, pregnancy. We screen at consultation.
How does this compare to fractional laser?
Different tools. Fractional CO2 is stronger per session but has more downtime (5–10 days). Microneedling + PRF is gentler per session but requires more sessions. For most aesthetic goals, microneedling + PRF achieves comparable results with less downtime and lower risk.
